Allegiant Healthcare of Phoenix

A June 2019 report states that Allegiant Healthcare of Phoenix, LLC has failed to provide adequate health care in two areas: pressure ulcer care and respiratory care.

In the first case, the resident arrived with a stage 3 pressure ulcer, but treatment as instructed by the care plan was not offered or documented properly on certain days, resulting in an unhealed sore.

In the second case, one resident required a specific type of respiratory care via a medical device and this was detailed in the care plan on admission. The medical device was to be used both day and night, but at some point had not been put on by staff.

There was no documentation that the device was used consistently as instructed. There was also evidence that this resident did not receive medication per physicians orders.

The Consequences of Untreated Bedsores

Pressure ulcers, which may also be referred to as bedsores, pressure sores and decubitus ulcers, progress quickly. Stage three pressure ulcers are essentially open wounds which do not heal on their own and require daily care.

These wounds are highly prone to infection and may not heal at all without surgery. The typical care goal is to prevent infection and promote wound healing.

Daily care typically involves:

  • leaning
  • Dressings
  • Debridement
  • Emollients
  • Nutritional support (IV)
  • Gauze
  • Pain management
  • Wound fillers
  • Repositioning the patient every 2 hours

Infections can occur at the wound site, but also throughout the body in the blood, joints, bones, and skin. The patient will likely be in a significant amount of pain and emotional distress is also common. Cancer, gangrene and significant tissue damage may also result.

Untreated bed sores can be fatal, and several wrongful death suits have been pursued in Arizona.

Failing to Follow a Care Plan can Constitute Neglect or Abuse

The Centers for Medicare and Medicaid (CMS) run nursing homes under a set of guidelines outlined in 42 CFR Part 483. .

All residents are to be given a thorough assessment and an official care plan is developed on admission. Unfortunately, a failure to maintain accurate and consistent records in accordance with this plan frequently leads to errors or worse.
